Basic Legal Requirements

Proof of Qualifying Conditions
- Must be suffering from or undergoing treatment for any of the qualifying conditions
- Written certification of qualifying conditions from an authorized physician
Complete the Paperwork
- Obtain all required documentation and complete all required forms from the provided checklist
- Complete online state application
- Pay the required fees online through Mastercard or Visa credit card, debit card, or prepaid cards
- Signed and dated department-approved forms from the application checklist provided
- Completed department-approved forms required from the qualifying patient checklist
