Once all required information and documentation are submitted, the patient will need to pay the non-refundable state application fee of $150 or the reduced fee of $75 for patients on Supplemental Nutrition Assistance Program (SNAP) (formerly known as food stamps) with a MasterCard or Visa credit card, debit card, or a pre-paid card to complete the application.
